We at Westhill want to make sure that your child is taken well care of.  In a case you child needs medical attention while they are in our care we need you to fill out the following form to help us make sure your child is well taken care of.  Thank you for entrusting your child into our care and allowing us to help in your spiritual growth.  Pleae note that contact with the parent or guardian will attempted to be made before any medical care is sought.
